The only app that allows image multi choice and rules based form construction. If you work with international clients or product images to assist in selection it's perfect.

Does exactly what it says it's going to do. Excellent app and intuitive to boot. Need a little help... but thats available.
Super easy to set up and does everything that we need a review app to assist in automating. Took 5 minutes to set up.
Great help desk... Got great assistance in setting up. Technically competent support and knew the complex answers. We go from boxes to pallets to 21 zones with different packaging and shipping requirements, and it works exactly how it's advertised.
Perfect app for uploading customer artwork. Support is on message when you are clear with the support you need. 5 stars. ... because it works and the support is there.
