We’ve tried every form solution out there, and SK: Advanced Form Builder is hands down the best for Shopify. It's completely free, supports image uploads, and integrates beautifully with our PageFly pages. The support is responsive, the form styling is flexible (we even added our own fonts and branding), and it works perfectly for our jewellery repair request system. Can’t believe this much functionality is offered for free. Highly recommended for any Shopify store looking for serious forms with serious flexibility!

I am truly surprised that this app is free. (*SEE UPDATE BELOW) The form building process is simple, the integrations are powerful, there is significant flexibility in the types of fields that can be used in the forms, and the forms can be relatively lengthy. The "back end" mailing options and ability to integrate with SMTP servers and data collection options such as Google Sheets was exactly what I was looking for. In short, this is the first app I have found that meets every wish I had when I decided to abandon the previous configuration I was using. For it to be literally exactly what I was looking for AND be free is just remarkable. *It is no longer free, at least, the features that I need are no longer free. I've spent several hours creating forms, making the connections for data to be transmitted to Google Sheets, setting up email notifications... now I am only able to use 2 of the forms I've been creating, Google Sheets is no longer available, and removing their branding is no longer an option (I was told that to remove their branding I had to write a review, which is why I did it in the first place)
We were looking for a form builder that integrates nicely with our branding, and SK more than delivers. The app is very easy to set up and customize, and includes many block elements for different data such as free text, radio buttons, star ratings, check boxes, etc. As a big plus, you can upload and submit files, and set conditional logic to create workflows depending on the answers on your form. You get email notifications when a form is submitted, but can also retrieve all submissions from the App admin. The support team is very responsive, and was quick to jump in and assist us during the implementation. Look no further if you need a form builder for your store, this one has everything you may need, and more.
This form is very useful and meets almost all of my needs. The customization options are excellent. I really like it—keep up the great work, team!
One of the most useful, flexible, and easy to use Form app for Shopify. We have been using it for a long time now for all of our website registrations and other submissions. The highlight for me is being able to embed it using a simple code which makes it super easy to use via meta-fields. 100% recommended.

Best Form Builder – Does Exactly What I Need I've been using the SK: Form Builder app on my Shopify store, and it's been great. Super easy to use with a clean Select-and-use interface. Incredible app that should be standard on Shopify. Great work guys! Very easy to use and create a custom contact form for my growing team with a new customer service function. I wanted the ability to send CS emails to my CS team while maintaining admin emails to my inbox. This is also at an incredible price - FREE! I would like an upgrade for flows to send sales emails to my sales team and order emails to my customer support team based on a form response, but for the price, this is simply a nice to have improvement! Great work team!

The functionality is very high for a free app. Has many of the features of the more expensive apps but it is all completely free. We use the google sheets integration a lot and find it very helpful. I would recommend learning some basic CSS for styling the forms. There are not many styling tools in the app itself.

This form is highly customizable! I can add headings between different sections of the form. Initially, I used the Shopify Form, but I have now switched to this app. I highly recommend it!
